THE FOLLOW BENEFITS ARE INCLUDED IN ​ OUR MEMBERSHIP HEALTH PLAN:​

Kari Ryan Dentistry is pleased to offer the Dental Membership Program to our patients not currently covered ​
by dental insurance. Our annual Dental Membership Program allows for a complete continuation of preventative care. ​

The health membership is not an insurance. The program is only offered to individuals and families not currently insured by a dental health insurance plan. All members must receive their care at Kari Ryan Dentistry. The program benefits are not transferable to another dental practice or dental specialty practice as our fees are separate.

PAYMENT IS ALWAYS DUE AT THE

TIME OF SERVICE

  • No deductibles, pre-authorizations, yearly maximums, exclusions, or waiting periods.​
  • It is the members responsibility to schedule and keep all appointments offered as part of the dental program.​
  • Membership is for 1 year beginning on the enrollment date. Membership will renew after one year of coverage at the patients recare appointment. ​
  • Annual Membership dues are due at time of enrollment and are non-refundable.​
  • Cash, check, or credit card payments are accepted for annual payment of the plan as well as payment for services.​
  • Payments for services not covered under the plan are due on the date of service to qualify for discounts detailed above.​
  • Membership must be current to receive discounts.​
  • Fees for dental services may change at any time.​

1 Panoramic X-ray (New Patients Only) Existing patients will get this at a discounted rate and it is taken every 3-5 years as needed 

2 Fluoride Treatments 

20% discount on all restorative services not covered by the plan if paid by cash or check (Zoom NOT included or any bleaching supplies) or 18% if paid by any form of debit or credit card 

$ 150 off Nightguards and 10% off of Sure Smile Clear aligner system. 

​2 Professional Dental Cleanings or 2 Periodontal Maintenance cleanings. (Additional Periodontal Maintenance cleanings are given with the below discount)
